Certified Copies

Additional certified copies of your marriage license are available upon request in-person or by mail.

Marriage License's are now a protected document.  We must have a completed Application to Obtain Vital Records Form filled out and signed.  Along with a copy of the front and back of the person(s) requesting drivers license.  Please call us at 1-435-734-3393 and we can email you a copy of the request form.  

    If requesting by mail, please include:

• Completed copy of the Application to Obtain Vital Records Form
• Name of person requesting copy
• Address of requestor                                                     
• Names of applicants at time of application 
• Date and place of marriage
• License Number, if possible
• $ 2.00 per uncertified copy
• $ 6.00 per certified copy
• Front and back copy of Drivers License
