fold the form in half and mail to Box Elder County Recorder/Clerk at 01 South Main Street, Brigham City, Utah 84302.

NOTE: You will want to print the form on one sheet of paper (a double side page).

Due to changes in Federal and State Election Laws we will no longer use the old punch-card systems.
The new touch screen voting system will now be used throughout the State of Utah.
In an effort to make the voting experience as easy as possible we will be demonstrating the new voting equipment.
